Language Assistance Services Notice

At Hilltop Healthcare Rehabilitation & Skilled Nursing, we are committed to ensuring that every resident and family member can fully understand and participate in care.

If you or your loved one have Limited English Proficiency (LEP), we provide free language assistance services, including:

  • Qualified medical interpreters (phone, video, or in-person when available)
  • Translation of important documents
  • Assistance during admissions, care planning, treatment discussions, and discharge


These services are provided at no cost to you.

How to Request Language Assistance

To request an interpreter or translation services, please:

  • Notify any staff member upon arrival
  • Contact our Admissions or Social Services team
  • Call our main line: (218) 625-6400


We will ensure a qualified interpreter is provided promptly.

Your Rights

You have the right to:

  • Receive information about your care in a language you understand
  • Request interpreter services at any time
  • Participate fully in decisions about your care
